7 June 17–19, 2021 2021 Fiesta NMDA Register online at www.nmdental.org Registration Rules & Guidelines Registration Rules & Guidelines - Please Read Carefully Before Registering The Primary Registrant must be the first name on the registration form. A dentist’s registration is not required for dental team members to register. Register before May 28, and all registration materials for in person attendees (i.e. badges) will be mailed to the Primary Registrant. All registrations received after May 28, are subject to the late fee. Late registration materials must be picked up at the NMDA Registration Counter inside the Albuquerque Convention Center during Fiesta NMDA registration hours. Please type or print clearly. Errors lead to delays in processing your registration. Select registration category and code for each person registering. Include appropriate fee according to registration date. Registration will not be processed until payment is received in full. Ticketed events are on a first-come, first-served basis. An Attendee registered as a “Guest” is not eligible to receive CE credit. Dentists may only register as dentists. On-site registration will be available. Ticketed events will be subject to availability. Avoid the line - register before May 28 NMDA / ADA 2021 membership must be current in order to pay member prices. On site attendees must were badges and must be visible at all times. All functions require this badge for admittance. Any individual transferring his/her badge to a non-registrant will have his/her badge confiscated and will be denied admission to the meeting. Lost or forgotten badges will only be replaced at the Registration Counter on-site. You may expect to wait in line for re placement badges. Please remember your badge! All registrations must be submitted on the registration form. No registrations will be accepted over the phone. Please keep a copy of the registration form for your records. NMDA reserves the right to deny Fiesta NMDA admittance to any individual. Attendee Checklist  Register by May 28 to save time and money! Register by mail, fax or on-line. Register only once.
