Pre-Registration Materials Badges and tickets are sent to registrants by mail up until May 28, 2021. Materials for those outside the U.S. are mailed up to May 21, 2021. Obtaining Official Documentation Attendees need to be pre-registered for on-site classes. Documents will be sent via email. First Aid Please go to the security desk outside of Hall 4 for assistance. Corrections/Changes/Additions Corrections, changes, and additions are processed at no charge after the original registration has been submitted. Registrants may make certain changes to their information and add classes by emailing narenas@nmdental.org as long as the changes are made according to deadline dates printed in this Registration Kit. Group Registration Group packages will be mailed to the address of the primary registrant. Guest Registration Guest registration designations are for family members (non- dental)—spouses and children. They will not earn CE credits and will not receive CEU forms. Refund Policy To obtain a refund, submit a written request, (accompanied by all badges and tickets) to NMDA, on or before May 28, 2021. All refunds will be processed on or before July 15, 2021. DATES & DEADLINES EARLY REGISTRATION DEADLINE May 28, 2021 CANCELLATION/ REFUND REQUEST May 28, 2021 HOTEL DISCOUNT REGISTRATION May 24, 2021 PRE-REGISTRATION DEADLINE May 24, 2021 Student Requirements 9 Students must have student ID card 9 Entry by paper form—faxed to 505-294-9958 or delivered to NMDA office by May 28, 2021 or on-site at the Annual Session 9 Student registration does not earn CE credits 9 Students will not receive CEU forms Ticketed Courses The only ticketed courses are those that require an additional fee for participation (except for the free Risk Management Seminar). Use the Ticketed Events table (on page 7) and enter the code on your registration form. All other courses do not require a ticket. Seating is on a first come, first served basis for all courses, so remember to arrive early. Lost Ticket/Lost Badge Lost tickets are replaced at the original cost. Lost badges are replaced at no additional cost. Badges All those attending the Annual Session must register and wear their badge throughout the Albuquerque Convention Center at all times. Entrance to the Exhibit Floor and classrooms are firmly restricted to attendees wearing a current badge. Badge Colors:  DDS  Hygienist  Dental Assistant  Business Staff  Technician  Exhibitor  Speaker  NMDA Staff  Student  Guest Regulations Audio, video taping, the use of photography, and the plac- ing of clothing on empty seats are prohibited in all Scientific Sessions. Smoking (regular cigarettes or e-cigarettes) is prohibited throughout the Albuquerque Convention Center. By attending the Annual Session, you consent to the NMDA’s use and/or reproduction of video/ photographyfor advertising, commercial, publicity, or any other business purposes of the NMDA. The NMDA will not be liable for any claims against you arising out of said use. You understand and agree that you will not be entitled to any financial or other compensation for said use by the NMDA.
